Payment Methods & Billing Process
We accept cash, credit card (MasterCard, Visa, American Express, Discover, or debit), or money order. We do not process credit or debit for costs less than $6.00. We do not accept personal checks.
We participate with most local and many national insurance plans. However, it is your responsibility to understand all the aspects of your insurance policy. We ask that you comply with your insurance’s pre-certification, prior-authorization, and co-pay mandates. We would be happy to help you with the process if you will call the office at least 48-hours before your appointment.
Our billing company ASC will submit claims to your insurance carrier and we will receive payments for the services rendered.
Please remember you choose the contract with your insurance company for payment, to which they decide how much of your medical bills will be your responsibility, rather than theirs. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. Insurance coverage is not a guarantee of payment. Therefore, if the bill is not paid in full by the insurance company, you will be responsible for the outstanding charges. In the event of a denial or cancellation of your insurance policy, you will be responsible for all charges. We are happy to help you with understanding the billing process if there is disagreement or misunderstanding about your insurance coverage.

Dr. Jeffrey Smith follows Title 76-19 in regard to medical records. According to Title 76-19:
A request for your medical records can take up to two weeks, but by law cannot take more than thirty days.
There is a charge of $1.00 for the first page and $0.50 for each page following. There is also a charge for postage. These fees must be paid before medical records can be copied and released.
We must have a signed release from the patient before any records can be released.
